Gerald baker OET letter

Mr Gerald Baker is a 79-year-old patient on the ward of a hospital in which you are Charge Nurse.

Mr Baker requires continuity of care and management, following his discharge today. He is currently recovering from a Left Total Hip Replacement surgery that he underwent in our hospital.

At present, Mr Baker shows good mobility, and he can walk short distances with the help of a wheeliewalker. Hospital is providing him a wheelie-walker, wedge pillow as well as a toilet raiser; the latter has to be returned after 2 weeks.

Post operatively, Mr Baker had felt disorientated, probably due to anaesthetic. In addition, his haemoglobin level had dropped temporarily; therefore, he should be watched as well as monitored for anaemia. He was commenced on occupational therapy: a range of motion, stretching and strengthening exercises, which needs to be continued.

Based on the above information, please continue the care for Mr Baker. Kindly monitor his medication regimen: Aspirin, 100mg in the morning and Panadeine Forte/co-codamol, 4 times, as required. Please maintain his skin integrity. On 21/9/2010, he needs to go for FBE and UEC follow-up tests at City Hospital Clinic, and his staples need to be removed.

  • Silvia worried she does not have enough breast milk and now gives extra formula feeds as well as breast feeding. Dylan difficult to bottle feed.

Dylan was breastfed the initial three weeks of birth and had no complications. Sylvia, then, switched to formula feed as per her mother-in-law’s advice as she got sick for a few days. She also mixes extra powder in formula feed, hoping to improve his weight, 4200gms.

Dylan is difficult to be bottle fed, and Sylvia prefers breast feeding over formula feed. However, she is concerned that she doesn’t have enough breast milk. Therefore, please teach Sylvia proper breast feeding technique and correct preparation of formula feed. If formula feed should be continued, boiled and cooled water needs to be added in the feed.

Dylan suffers from constipation, and his stool is hard every three days; the urine appears dark. He only had 3 wet nappies in the last 24 hours. Since he had become unsettled during summer wave, he needs to be kept hydrated and cool in hot weather. His reflexes and vitals are normal, but he is slightly lethargic.

During hospitalisation, Mr Woods was treated with IV antibiotics, analgesics, and antithrombotic agents. His graft site, left leg, was dressed.  Daily physiotherapy, deep breathing, and walking for 4-5 mts, 4 times a day, were started. He was administered Brufen for chest pain. His recovery was good.

Mr woods has been hypertensive since 2011. He also suffers from Arthritis and high cholesterol since 2007 and 2009, respectively. He follows his medication regime for the same.

Mr woods is the only care taker for his wife, who is a registered blind. His daughter, with her family, lives abroad.

Based on the above, kindly receive him and provide on-going care. His wound over the chest and graft site requires daily monitoring and dressing twice every week. His sutures need to be removed at the end of 2 weeks. He needs to continue his deep breathing exercises and should gradually increase his daily walk. He must avoid heavy lifting. Prior to his discharge, please arrange a carer and provision for meals at his residence. His follow-up appointment will be on 07/06/2021.

Given the fact that international travel can be quite an expensive endeavour, I’ve decided to explore territories within India. I’ve been visiting one place after another, making the most of the diverse cultures and landscapes the country offers.

So far I’ve covered Delhi and Tamil Nadu, ticking off some of the top destinations on my bucket list without a second thought. The next place on my list is none other than Nagaland. I’ve heard incredible things about it, especially from the B.B.C.. What fascinates me the most about Nagaland is its rich history and cultural heritage.

I’ve learned that the people there were once headhunters and warriors, engaged in conflicts that have shaped their traditions. Additionally, the Kohima War Cemetery, which commemorates the sacrifices made during the Second World War, is a place that I’m eager to visit. Another attraction that excites me is Zuko Valley, known for its abundance of flowers and stunning natural beauty. The picturesque landscapes and serenity are what draw many visitors to the region. But it’s not just the nature, it’s the people of Nagaland too.

The Naga tribes embody unique beauty through their diverse culture and traditions. With at least sixteen tribes, each speaking a different language, Nagaland is a tapestry of fascinating stories waiting to be discovered.

I’ve been contemplating this destination for quite some time, and I’m planning to go by next December as they celebrate a major festival every December twelfth. I’m considering taking a flight, even though there are other options like trains. The challenge with trains is the considerable time it takes, at least three days, whereas flights, although indirect, would be more time efficient, taking approximately sixteen to twenty hours. I have many friends from Nagaland who have shared positive feedback, and I’m eagerly looking forward to experiencing it all myself.

One of the most unusual foods I’ve ever tried was fried grasshopper. I had the opportunity to taste this unconventional delicacy during a visit to Nagaland, my wife’s hometown in Northeast India. It was an experience that left me with a mix of initial disgust and later intrigue.

Grasshoppers are typically consumed by the local community only during the rice harvesting season. They are considered a seasonal treat, as consuming them at other times can be potentially dangerous due to their susceptibility to pesticides and other toxins. The villagers have a unique method of catching these creatures. They venture out into the fields at night with torches, which attract the grasshoppers, and then they skilfully catch them.

The grasshoppers are cleaned and prepared for cooking by removing their legs and wings. They are then seasoned and marinated before being fried to a crispy texture. The thought of eating insects was initially off-putting, and I was quite disgusted at the sight of these creatures. However, my curiosity got the better of me, and I decided to give it a try.

Upon my first bite, I was surprised by the intriguing taste. The fried grasshoppers had a delightful crunchiness and a unique blend of flavours from the seasonings. It was unlike anything I had ever tried before, and I found myself wanting to have more. The locals in Nagaland enjoyed these snacks, and their enthusiasm was infectious.

While I may not have been entirely comfortable with the idea of eating insects, the experience broadened my culinary horizons and gave me a deeper appreciation for the diversity of food cultures around the world. It taught me that what may seem unusual or even unappealing at first glance can turn out to be a surprisingly delightful gastronomic adventure.

If the opportunity arises in the future, I would certainly be open to trying fried grasshoppers again. It’s a unique and memorable food experience from Nagaland, my wife’s place, that I wouldn’t mind revisiting.

A historical event that I find immensely significant is the Indian independence movement, which culminated in India gaining its independence from British colonial rule. This pivotal event occurred in the mid-20th century and has had profound consequences not only for India but for the world at large.

The Indian independence movement reached its zenith on August 15, 1947, when India finally achieved its long-awaited freedom. This event marked the end of nearly two centuries of British colonial rule in India. The struggle for independence was marked by various key details, including non-violent resistance led by figures like Mahatma Gandhi and the tireless efforts of countless individuals who believed in the cause.

I’ve read, learned, and heard about many distinguished individuals who have done valiant and admirable things in their lives. Among these, Mahatma Gandhi’s life story particularly captivates me. Gandhi is celebrated for his pivotal role in Indian politics, especially in the monumental struggle to overthrow the British colonisers from Indian soil.

I first learned about Gandhi through history lessons in school and later through various books, documentaries, and films that detailed his life and contributions. His name is often cited among a distinguished few global leaders who have made an indelible impact on society and the world through peaceful means.

What I admire most about Gandhi, among his extraordinary qualities and unparalleled achievements, is his unwavering dedication to non-violence even in the face of brutal oppression. Gandhi’s ability to mobilise millions without wielding a sword or pulling a trigger, advocating instead for peaceful resistance and civil disobedience, was both remarkable and inspiring. He embodied and practiced love, compassion, and tolerance, principles that have left an indelible mark worldwide. His teachings on simplicity, self-reliance, and humility continue to resonate deeply with people around the world.

Gandhi’s non-violent and humble lifestyle has had a profound impact on me, illuminating my path and urging me to eschew violence and pride. I am confident that his legacy will continue to inspire and enlighten many more lives in the years to come. If only people, societies, and even countries endeavour to embody just a fraction of the values and principles Gandhi exhibited, the world would undoubtedly be a better place.
